Social workers with empathy and the drive to make a difference, finding creative ways to support people to achieve what they want to and live as independently as possible in a place they call their home.

We need a permanent, part-time, Social Worker to work across Lancashire as part of our Adult Services Older Adults and Physical Disability Community Teams.

As part of your exciting new role, you will be expected to undertake strength based, outcome focussed assessments and support planning, ensuring that the residents of Lancashire receive the right amount of support.

You will play a vital role in supporting people to access local services, maintain independence, promote health and wellbeing and to live independently.

A key part of the role includes working alongside other agencies, professionals, and informal carers, providing a holistic service.

In return for your commitment and hard work, you will work alongside supportive managers and offered various training and progression opportunities.
