Job summary

Liquid Personnel is seeking a Senior Social Worker to join the Community Team in Shropshire, supporting adults across the North Shropshire locality. The role focuses on older adults, people with disabilities, and complex care needs with strengths-based, person-centred interventions to promote independence.

You will manage a complex caseload, deliver high-quality assessments, care planning, safeguarding, and professional social work support to help people stay at home and in their communities

Qualifications

• Qualified social worker with SWE registration and substantial post-qualification experience.
• Experience undertaking complex assessments, reviews and risk management.
• Knowledge of Care Act 2014, Mental Capacity Act 2005, adult safeguarding legislation.
• Hospital discharge and community-based social work experience is desirable.

Responsibilities

• Undertake Care Act 2014 assessments, reviews and support planning.
• Manage a complex caseload and complete statutory duties to a high standard.
• Apply strengths-based practice to maximise independence and participation.
• Develop and review person-centred care and support plans.
• Contribute to safeguarding enquiries and multi-agency procedures.
• Undertake Mental Capacity Assessments and Best Interests decisions.
• Support hospital discharge and coordinate community services.
• Work with health professionals, carers, families and agencies.
• Assess and manage risk while balancing rights and autonomy.
• Promote service user involvement in assessment and planning.
• Maintain accurate records and reports in line with requirements.
