We are a Local Authority working for the community, improving the lives of our residents and we are looking for a motivated and talented Senior Social Worker to join our Advice and Wellbeing Hub SW Pathway.
As a Senior Social Worker, you will play a key role in delivering high-quality advice, early intervention, and support to those accessing the hub. You will provide professional leadership, undertake complex assessments, and work collaboratively with partner agencies to promote wellbeing and safeguard vulnerable individuals.
The Advice and Wellbeing Hub comprises a Social Work and OT pathways. The team takes all the new referrals into Adult Social Care for people 18 years upwards. It is a busy, supportive and friendly team, working with people of all ages and backgrounds and their families and carers. We use a Conversations Count strengths-based model to promote choice and control and achieve excellent outcomes. You will work with individuals who have been referred for support for up to 6 weeks, to maximise their independence in their own homes.
About the Role:
Your duties as Senior Social Worker include providing supervision to the Social Workers and Social Care Coordinators within the team and undertaking joint visits with other members of the team to support knowledge sharing and skills development. You will also support the Team Manager in some aspects of managing the team as a whole and service development.
The role involves you undertaking work using the Conversation Counts model, working on prevention and early support. Using your social work expertise, you will conduct Care Act assessments, safeguarding investigations, and Mental Capacity assessments, as well as facilitate best interest meetings.
This role involves working with adults with diverse needs, including individuals with learning disabilities, physical disabilities, older adults, and those affected by substance or alcohol misuse. Additionally, you will provide information, advice, and support to carers, helping them navigate available resources and services. This position offers an excellent opportunity to develop your skills and experience in Adult Social Care, delivering high-quality, person-centred interventions to individuals in their own homes.
