Position: Senior Social Work Practitioner – Transitions Team
Location: Birmingham, UK
Salary: £33.33 Hourly
Type: Locum
Duration: 3 months

This is an exciting opportunity for a skilled Senior Social Work Practitioner to join our dynamic Transitions Team in Birmingham for a short-term project. Your expertise will help support young people aged 18-25 with disabilities, ensuring they receive the best possible care and guidance into adulthood. This full-time position offers the chance to oversee a dedicated team of social workers and make a genuine impact on the community.

Responsibilities
  • Lead a team of 9 social workers in managing day-to-day responsibilities, including support planning, budget requests, reviews, and mental capacity assessments.
  • Participate in meetings for best interest decisions, Education Health and Care Plan discussions, section 117 reviews, and Complete Continuing Healthcare (CHC) assessments.
  • Ensure defensible decision-making in a fast-paced environment, guiding your team towards effective results.
  • Provide supervision and address any performance issues to maintain high standards of service delivery.
  • Conduct home visits and assessments throughout the city, supporting young people in various locations.

Working arrangements
