Job summary

Belfast Health and Social Care Trust seeks a Primary Care Social Worker: Senior Practitioner (Band 7) to join GP practice teams in West Belfast. Qualifications

• Hold a professional Social Work qualification (Degree, Dip SW, CQSW or equivalent).
• Hold current NISCC registration or be eligible for immediate registration.
• Minimum five years post-qualifying social work experience at band 6 or equivalent.

Responsibilities

• Provide social work assessments and interventions in primary care teams.
• Use social prescribing and develop group-work interventions where appropriate.
• Provide line management and supervision for social work assistants in the primary care team.
• Advise MDT colleagues on pathways and thresholds for intervention across Trust services.
• Contribute to strategic planning and resource development for population health.
