Main duties of the job

From assisting multi-disciplinary teams in patient assessment to guiding clinical decision-making and treatment for level 1, 2, and 3 patients, your role will be essential in ensuring top-notch care delivery.

What You'll Do:

  • Educational Leadership: Deliver a dynamic and flexible program of inter-professional, evidence-based critical care education, aligned with our team's education strategy.
  • Innovative Services: The Critical Care Outreach Team (CCOT) oversees our Nurse-led services, such as Non-Invasive Ventilation for acutely unwell patients.
  • Leadership in Working Groups: Lead and collaborate on various working groups, including those focused on sepsis, deteriorating patient care, acute kidney injury, NIV, and tracheostomy management.
  • Risk Evaluation and Management: Work closely with risk management, evaluating and contributing to serious incidents and Patient Safety Incident Reporting and Feedback (PSIRF).

Detailed job description and main responsibilities

Communication:

  • Liaise closely with the Deputy Matron and Matron for Critical Care Services.
  • Professional accountability is to the Deputy Matron and Matron for Critical Care Services.
  • To facilitate on the development of standards, guidelines, policies and share good practice within the multidisciplinary.
  • Work collaboratively and maintain effective communication and exchange of clinical information with all members of the multidisciplinary teams, across all specialities, providing care for the critically ill and/or deteriorating patient.
  • Clinical accountability is to the Consultant responsible for the patients care.
  • Will be required to communicate sensitive, complex and unpleasant news to patients and their families.

Analytical:

  • To act as a proficient practitioner utilising advanced skills in clinical decision making, clinical assessment, and investigation, suggest differential diagnosis, treatment, planning and evaluation for care of the critically unwell or deteriorating patient, to improve patient outcomes and reduce further risk to health.
  • Manage complex ethical and professional issues, making informed judgements.

Planning and organisational:

  • Provide clinical expertise to facilitate the safe and efficient intra-transfer of the critically unwell patient.
  • To attend and provide critical care expertise at trauma and cardiac arrest calls.
  • To provide expert clinical assistance to nursing and medial colleagues in the implementation and management of ward-based non-invasive ventilation.
  • To provide expert clinical assistance to nursing and medical colleagues in the management of tracheostomies and Laryngectomies.
  • To act support the Hospital at Night co-ordinator, help prioritising and allocating the workload of the Hospital at Night Team.

Service development:

  • Collect, analyse and report audit data to provide information for the evaluation and development of the critical care service, improving quality and outcome.
  • To work in conjunction with the Deputy Matron and Matron in contributing to the Strategic planning of the service and new initiatives annually.
  • To work in conjunction with the Education Lead for Outreach, Deputy Matron and Matron in contributing to the education strategy for nursing and other healthcare care staff.
  • Raise the profile of Critical Care Outreach locally and nationally by attending, representing and presenting at study days, network meetings and conferences.

Responsibility for people or training:

  • Encourage professional development through role modelling, compassionate leadership and sharing expertise in caring for the critically ill or deteriorating patient, improving diagnostic reasoning skills and critical thinking.
  • Promote a culture of improving health and safety by raising concerns, conducting risk assessments and ensuring the appropriate actions.
  • Provide specialist knowledge and opinions on complaints and serious incident panels/investigations.

Person specification

Role requirement:

  • Registered RN Part 1(adult)
  • Evidence of level 6/7 health related course
  • Post Registration ITU course
  • Minimum 6 months band 6 role in critical care
  • 3 years critical care experience
  • Previous H@NT experience
  • CCOT experience
