Job summary

Lancashire & South Cumbria NHS Foundation Trust invites applications for a Band 6 Registered Mental Health Nurse to join the Older Adult Community Mental Health Team based in Charnley Fold, Bamber Bridge. You will manage caseloads, undertake assessments, develop care plans and work with a multidisciplinary team to deliver person-centred, recovery-focused care.

The role operates Monday to Friday, 9:00am to 5:00pm, with opportunities to collaborate with primary care, social care and community

Qualifications

• RMN with current NMC registration; CPD evidence; Practice Supervisor/Assessor qualification or willingness to undertake.
• Mental health knowledge including older adults; safeguarding; risk management.

Responsibilities

• Undertake comprehensive mental health assessments and care planning for older adults.
• Coordinate care and manage risk within a multidisciplinary/team setting.
• Deliver evidence-based interventions and contribute to reviews and MDT discussions.

Education
