Job summary

Central Cheshire Integrated Care Partnership (CCICP) in Crewe is recruiting an experienced nurse/physiotherapist for Adult Respiratory Medicine. The post covers community and in-reach services, with a seven-day service focus on long-term respiratory care and home oxygen assessments.

You will work autonomously within a multidisciplinary team to assess, manage and review patients, prioritise workload, and maintain high standards of patient care in both home and hospital environments.

Qualifications

• Post registration experience in a relevant field for a minimum of 3 years.
• Autonomous practitioner with ability to work across community and acute settings.
• Registered professional with HCPC/MNC and current UK practice.

Responsibilities

• Provide specialist respiratory care to patients in community and acute settings.
• Conduct face-to-face and telephone assessments and reviews.
• Lead Oxygen assessment clinics and coordinate care pathways.
