We are currently looking for a Senior Reablement support worker to join our Intermediate Tier Services in Warrington.

The role

This is a fantastic opportunity to be a part of shaping integrated care working with partners to improve outcomes and experiences for the people of Warrington. We are looking for a value led, dynamic, outcomes driven and compassionate individual to join the service at a time of significant opportunity. We are well on our journey to delivering collaboration across Health and Social Care to improve outcomes for those living with frailty in Warrington.

To Identify and assess the needs of the people in Warrington to support them to live well, age well and to remain as independent as possible in the place they call home. Supervising a team of Support Workers to deliver a quality service and ensuring the individual needs, wishes and goals of the people we support are met.

What's needed?
  • Able to commit to a rota which includes days, evenings, weekends and bank holidays
  • Flexible approach to work with a willingness to cover for colleagues' annual leave, sickness etc.
  • Flexible approach to work and the ability to respond to varied needs
  • Current Driving Licence and have access to a vehicle.
