Hampshire and Isle of Wight Healthcare NHS Foundation Trust in Southampton is seeking a Mental Health Nurse, Social Worker or Occupational Therapist to join the West PCN primary care Mental Health Team, working in GP surgeries with adults and older adults with mild to moderate mental health needs. You will operate within your scope of practice.

The role focuses on triage, early assessment, brief interventions, signposting, and collaboration with Solent Mind and other practitioners.

Qualifications

  • Registered MH nurse, social worker or OT.
  • Computer skills.
  • 2 years post qualifying experience.
  • Mentorship course.
  • Clinical supervision training.

Job summary

We are seeking a Mental Health Nurse, Social Worker or Occupational Therapy team leader to join our developing primary care Mental Health Team in West PCN. You will be based within GP surgeries and working with service users of adult working age including older persons with mild to moderate mental health difficulties. You will work within scope of practice so if have worked within specialities these skills will be transferable

Only open to applicants from Hampshire & Isle of Wight NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service, Isle of Wight NHS FT or Hampshire Hospitals FT.

Main duties of the job

You will contribute to the triage service; offering early assessment, brief interventions and signposting for people with mental health needs in their local communities. You will play an active part in the development of systems and processes which support care delivery within these teams. You will require good communication skills and be able to build positive partnerships with key stakeholders in primary and secondary care. You will work collaboratively with Solent Mind and have peer recovery workers work alongside practitioners in the surgery.
