An Exciting opportunity has arisen for an existing PCDP to have a permanent band 6 post within the North and East Locality
This post will be support the existing band 5 PCDPs in the North and East ICA, offering supervision and guidance and working alongside the leadership team.
The successful applicant will continue with their current caselist and base, but will inreach to the North and East ICA as the band 6 PCDP lead.
The post holder will be based and embedded within a Primary Care Network (PCN) working with GPs, Dementia Together Service, practice nurses and other Additional Roles e.g., social prescribers, health and wellbeing coaches, care co-ordinators, as part of the primary care Multidisciplinary Team (MDT).
