Job summary

Thurrock Council is seeking a Senior Practitioner for the Urgent Community Response Team (UCRT) in the United Kingdom. The role leads day-to-day practice, undertakes complex assessments, and manages risk to provide rapid, person-centred interventions that reduce hospital admissions and support safe discharges in the community.

You will supervise colleagues, work with health partners and local agencies, and contribute to service development in a fast-paced 7-day service.

Qualifications

• Qualified social worker registered with Social Work England.
• Experience of safeguarding and risk management in adult social care.
• Ability to work across multi-disciplinary teams and manage complex caseloads.

Responsibilities

• Provide professional leadership and oversight within the UCRT.
• Undertake complex assessments and manage risk to ensure safe, timely interventions.
• Coordinate rapid response support to prevent hospital admissions and support discharges.
• Supervise and mentor team members, providing guidance and reflective practice.
• Collaborate with health partners and other agencies for integrated service delivery.
• Manage a dynamic caseload in a 7-day service with prioritised urgent needs.
• Contribute to service development and continuous improvement.
