Job summary

Provide Community is seeking a Band 7 nurse or paramedic to lead our Urgent Community Response Team (UCRT). You will provide rapid, patient-centred assessments and treatments in people’s homes to prevent hospital admissions.

As clinical lead you will provide senior clinical leadership, oversee nursing procedures and support the team in managing complex cases within the home environment, coordinating with ambulance, GP and social care partners across Essex.

Qualifications

• Senior clinical leadership experience in community care.
• Nurse (adult) or Paramedic with registration.
• Ability to lead rapid assessment and treatment in home settings.
• Commitment to high quality, safe patient care.

Responsibilities

• Provide senior clinical leadership within the UCRT team.
• Undertake clinical duties and nursing procedures.
• Support the team to manage more complex patients at home.
• Improve outcomes for frailty and multimorbidity; reduce hospital admissions.
• Coordinate care with partners to optimise service delivery.
