Contract: 3 months
Location: Plough Lane, Hereford. To cover Herefordshire.
Hours: Full time (37 per week)
Pay: £38 per hour Ltd/Umbrella

Herefordshire Council Adults & Communities Care Act Assessment Team is currently seeking a Locum Senior Practitioner to conduct short-term assessments and support planning with service users across Herefordshire, particularly those who have recently left hospital via Discharge to Assess and may require ongoing services. This is an exciting opportunity to join our team. We are looking for full-time, enthusiastic staff who thrive in fast-paced environments, work within set timescales, and are committed to supporting service users.
We seek a forward-thinking Senior Practitioner who is a qualified, experienced social worker, registered with Social Work England, with strong knowledge of case management, excellent organizational skills, and effective decision-making abilities, including positive risk-taking and conflict resolution.
The ideal candidate will have a solid knowledge base in adult social work, strong interpersonal and organizational skills, and a good understanding of social care practice and legislation. You should embody our values of person-centered, strength-based, safe, and effective responses, with a commitment to achieving the best outcomes for individuals supported. You will work independently to meet assessment and review requirements, support staff through supervision, and oversee safeguarding, allocations, triaging, and assessment completion.
Experience in hospital discharges, Continuing Healthcare, Mental Capacity Act (2005) assessments, mental health, learning disability, physical disability, and older adults is desirable. You may also manage a small caseload.
You will receive regular supervision and opportunities for professional development. Given Herefordshire's rural setting, the ability to travel is essential.
