Senior Practitioner – Integrated Triage & Assessment Services

This position qualifies for a market supplement payment of £200 per month. Please note this additional payment will be reviewed on 31 March 2025.

We are recruiting a permanent, Senior Practitioner Social Worker for the Integrated Triage and Assessment team, Porth Gofal Service.

This role offers an exciting opportunity for a dynamic and committed professional to work in a multidisciplinary environment supporting vulnerable individuals and their families in hospital and community settings.

Porth Gofal is the integrated decision-making hub for Ceredigion, responsible for triaging cases and determining the level of need and best outcomes for individuals.

Porth Gofal ensures that citizens receive the most appropriate intervention or are directed to early help or specialist services.

What Porth Gofal provides:
  • A responsive service to citizens in Ceredigion
  • Strength-based needs assessments focusing on service user choice and control, using the Signs of Safety framework
  • Reduced time for social care professionals to determine eligibility for statutory services
  • A legislatively compliant service meeting the statutory requirements of the SSWBA
  • A safe service that identifies vulnerable and at-risk citizens and responds effectively
What Porth Gofal achieves:
  • Provides information, advice, and assistance about community health and social care services for adults, improving community connection
  • Encourages a balanced assessment of needs, ensuring citizens are central to conversations and decisions, including the 'what matters' approach
  • Multidisciplinary triaging of referrals to identify the appropriate professional response, e.g., physiotherapist, occupational therapist, or social care worker
  • Provides consistent, coordinated assessments for care and support
  • Enhances information flow between services for faster decision-making
  • Reduces hospital admissions through rapid, wrap-around reablement responses
  • Supports timely discharges from hospital to reduce delayed transfers of care

A commitment to integrated working with a focus on positive rehabilitation outcomes is essential.

Travel throughout the county is required.

Applicants must have at least two years post-qualifying experience, including staff supervision.

A bilingual policy is in operation; candidates without Welsh language skills will be supported to achieve the ALTE standard within two years.
