We are looking for qualified Senior Practitioners to join the Personalised Healthcare Commissioning (PHC) service to work with our Teams in supporting ICBs in providing support for patients in receipt of health funding.
The post holder will play a key role in supporting the Personalised Healthcare Commissioning service across the region. Working within the remit of the National Framework for Continuing Healthcare and other NHS funding streams, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service.
You should be well-versed in dealing with complex and sometimes contentious information.
The post holder should hold a full UK driving licence and have access to a vehicle to fulfill the requirements of this post.

Main duties of the jobThe postholder will be expected to lead on Continuing Healthcare assessments across the responsible ICB area.
You will support and advise colleagues regarding NHS Continuing Healthcare Funding, ensuring current practice is embedded into clinical practice, promoting best practice and compliance with the Department of Health processes.
The role will be responsive to ICB priorities and expectations whilst working on their behalf and representing them through work with NHS Continuing Healthcare.

Detailed Job Description And Main ResponsibilitiesJob responsibilities
- Leading on complex cases within a team of Personalised Healthcare Assessors, ensuring high-quality service delivery, supporting and mentoring staff for continuous improvement.
- Managing a caseload of patients and contributing to performance improvement, taking a lead for specific areas as agreed.
- Managing a defined caseload for those with complex health needs.
- Managing staff in line with regional and local HR policies.
- Facilitating and contributing to multidisciplinary assessments and patient reviews, following standard operating procedures.
- Participating in the recruitment of junior staff.
- Supporting the PHC team to ensure workstream plans are fulfilled and supervised effectively.
- Ensuring compliance with the NHS Continuing Healthcare National Framework.
- Deputizing for the team leader when required.
- Supporting resolution of complaints, appeals, and disputes.
- Raising issues of concern with Senior Management.
- Mentoring and supervising PHC Associate Practitioners, Practitioners, and students.
- Monitoring workstreams and quality assurance of processes.
- Investigating variances from targets and implementing solutions.

Person SpecificationEssential Criteria
- Healthcare Professional Registration, i.e., RMN, RNLD.
- Extensive knowledge of specialist areas and the continuing healthcare process, plus MDT working experience.
- Negotiation and conflict management skills, with the ability to influence.
- Effective workload management and decision-making skills.
- Strong communication skills.
