Job Title: Senior Practitioner - Complex Care Team

Location: London Borough of Hackney

Hourly Rate: Up to 36 per hour

Are you an experienced and highly skilled social worker looking for a challenging yet rewarding opportunity? The London Borough of Hackney is seeking a dedicated and passionate Senior Practitioner to join our Complex Care Team.

About the Role: As a Senior Practitioner, you will play a crucial role in providing high-quality social work services to individuals with complex care needs within the community. You will manage a diverse caseload, offer expert guidance and support to less experienced colleagues, and contribute to the development and implementation of best practices within the team.

Key Responsibilities:
  • Manage a complex caseload, undertaking comprehensive assessments and developing person-centred care plans.
  • Provide leadership, supervision, and mentoring to social workers within the Complex Care Team.
  • Contribute to the continuous improvement of services, ensuring adherence to statutory requirements and local policies.
  • Collaborate effectively with multi-disciplinary teams, external agencies, and service users' families/carers to achieve positive outcomes.
  • Participate in safeguarding adults processes and uphold the highest standards of professional practice.

About You:
  • A qualified Social Worker with current registration with Social Work England.
  • Demonstrable experience in a complex care or adult social care setting.
  • Proven experience of supervising or mentoring other social workers.
  • Excellent knowledge of relevant legislation, including the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983.
  • Strong assessment, analytical, and communication skills.
  • A commitment to promoting independence, choice, and control for service users.
