About the Role
You will be responsible for supporting the CHC Lead in the effective delivery and management of the NHS Continuing Healthcare (CHC) pathway. You will undertake joint assessments and reviews in partnership with the ICB to determine CHC or Joint Funding eligibility under the 2022 National Framework and the Care Act 2014.
This role requires a practitioner who can confidently present evidence, challenge decisions, and lead on disputes regarding CHC outcomes.
Responsibilities
Conduct CHC assessments, Checklists, and Decision Support Tools (DSTs)
Collaborate with the ICB to determine CHC and Joint Funding eligibility
Support the management of the CHC process from referral to outcome
Assist in monitoring the financial impact of CHC outcomes
Present arguments effectively using Nature, Intensity, Complexity, and Unpredictability of health needs
Provide face-to-face assessments and office presence at least twice per week in Hereford
Document and follow up cases from home working days
About You
Are you a confident CHC practitioner with strong understanding of health and social care frameworks and a passion for advocacy? If so, this role could be an excellent fit for you. You will need:
Proven experience in CHC assessments and disputes
Excellent presentation, communication, and negotiation skills
Strong understanding of the 2022 National Framework and Care Act 2014
Experience gathering and presenting evidence to support CHC eligibility decisions
Experience using Mosaic (desirable, but training available)
HCPC registration (essential)

Contract length: 3 months (with potential extension)

Location: Plough Lane, Hereford, HR4 0LE
Working pattern: 37 hours/week, hybrid (2 days in-office required)
