Discharge to Assess Community Team North Pathways
An exciting opportunity has arisen to join the Hospital Sector Discharge to Assess Teams based within various locations.Successful candidates will work across busy Multi-Disciplinary Teams to facilitate social work practices.
As a Senior Practitioner your focus will be to support and coach the Social Care workers within your cluster / team to be the best in class, with a clear focus on culture change, to improve on performance, quality of practice and to deliver improved outcomes within the high support and high challenge environment, whereby enabling the workforce to own and drive their own performance.
Your main responsibility will be to supervise and oversee the management of your cluster, and the overall team delivery to have a One Team approach. You will support your cluster through regular supervision, ensuring the multiple complexities within caseload are managed in a timely manner, efficiently and effectively. In addition, you will undertake casefile audits, promoting a culture of shared learning and improvements with a strong one team ethos that focuses on team performance, community development and partnership working.
