Bromley GP Alliance Ltd in Bromley is seeking an Experienced Practice Nurse (ARRS) to work across the Crays PCN, delivering high-quality nursing care across six GP practices. The role covers chronic disease management, cervical screening, vaccination programmes and housebound care, with emphasis on evidence-based, patient-centered care and MDT collaboration.

The post is permanent, part-time, with pay £20.44 to £24.61 per hour depending on experience, subject to NHS terms.

Qualifications

• Registered Nurse (Adult) with current NMC registration.
• Diploma or Degree in Nursing.
• Cervical Screening Qualification and current competency.
• Immunisation and Vaccination Training.
• Continuing professional development and participation in CPD.

Responsabilités

• Undertake and manage long-term condition reviews.
• Develop personalised care plans for patients.
• Support medication optimisation through review and monitoring.
• Identify deterioration or unmet need and refer appropriately.
• Deliver cervical screening appointments and ensure informed consent.
• Deliver adult and childhood immunisation programmes.
• Provide nursing care to housebound patients across the PCN.
• Maintain accurate clinical records and contribute to governance.
