City Health Partnership Ltd (CHPL) is seeking a highlymotivated and enthusiastic Senior Practice Nurse with a V300 IndependentPrescribing qualification to join our team. The role involves the clinicalassessment and management of patients primarily within a general practicesetting, focusing on long-term condition management, including asthma,diabetes, COPD, and hypertension.
Consultations will be conducted face-to-face, by telephone,or via digital platforms, managing undifferentiated illness and acutepresentations related to long-term conditions. The successful candidate willhave a strong background in general practice nursing, with a thoroughunderstanding of QOF (Quality and Outcomes Framework) requirements,immunisation programmes, and current health campaigns.
The Senior Practice Nurse will ensure the highest standardsof clinical care across CHPL, working within their professional boundaries andsupporting the implementation of organisational policies and procedures. Theywill work closely with the Clinical Team Leader to support and improve serviceprogression, contribute to service improvement initiatives, and mentor juniornursing staff. This role demands advanced clinical skills, includingindependent prescribing, to provide comprehensive, holistic patient care.
The service opening hours are Monday-Friday 8am-6.30pm andit is expected contracted hours will be worked during these times.
The post holder is accountable for the delivery of safe, high-quality, and effective clinical care within primary caremedical services. They will provide leadership and support to all nursing staff, collaborating closely with otherleaders and the multidisciplinary team to achieve service priorities. Clinically, the role is focused on implementingevidence-based practice to meet the needs of a diverse patient population, ensuring optimal health outcomes.
