A leading healthcare provider in England is looking for motivated and experienced Band 6 Physiotherapists to join their Rapid Assessment and Discharge Service team. You will engage in delivering high-quality, person-centred care for older adults, contribute to service development, and collaborate with a multidisciplinary team to improve patient outcomes. Flexible working opportunities are available within a supportive environment. Full-time and part-time positions are open, closing on October 1, 2025.
Due to the continued success of UHL’s Frailty Same Day Emergency Care (SDEC) pilot, we are excited to offer opportunities for motivated and experienced Band 6 Physiotherapists to join our innovative Rapid Assessment and Discharge Service (RADS) team.
Frailty SDEC is a forward-thinking model of care designed to deliver timely, multidisciplinary assessments and interventions, outside of the Emergency Department, to older adults presenting with frailty, enabling same-day discharges and reducing unnecessary admissions.
As part of the RADS team you’ll play a key role in delivering high-quality, person-centred assessments across the Emergency Department, Acute Assessment Units as well as Frailty SDEC. You will work collaboratively with a large multidisciplinary team (MDT) to provide comprehensive care supporting safe, timely discharges with robust therapy input and community liaison. Furthermore, as a team we are passionate about promoting independence and preventing deconditioning for all patients across the Emergency Floor through early therapy intervention, effective MDT working and teaching.
Our new strategy, developed with the support and feedback of colleagues, patients, and partners, is our compass for the next seven years (2023-2030).
• Deliver timely, person-centred assessments and interventions for older adults presenting with frailty within the Emergency Department, Acute Assessment Units, and Frailty SDEC.
• Collaborate with a large multidisciplinary team (MDT) to support safe, timely discharges with robust therapy input and community liaison.
• Promote independence and prevent deconditioning through early therapy intervention, MDT working, and teaching across the Emergency Floor.
• Contribute to service development, quality improvement, and teaching as part of a forward-looking RADS team.
We have four primary goals:
We will embed health equality in all we do, taking active steps to reduce avoidable differences in healthcare, working in partnership with communities.
Our strategy is underpinned by new values and we will work to ensure they are an everyday reality for all:
