Senior Physiotherapist (Intermediate Care)

Key Responsibilities:
- Be professionally and legally accountable for all aspects of your own and delegated work.
- Work within codes of practice and professional guidelines.
- Undertake holistic assessments, including those with complex presentations and multi-pathologies, utilizing specialist clinical reasoning skills.
- Use evidence-based practice to ensure effective and appropriate clinical care.
- Promote independence, aiming to avoid hospital admission and reduce inpatient length of stay.
- Ensure patients receive appropriate care in the most suitable setting.
- Organize and manage your own time, delegating work appropriately.
- Conduct risk assessments and health and safety assessments, reporting any risks promptly.
- Gain patient and carer consent for all interventions.
- Communicate effectively with patients, carers, and colleagues, using tact and persuasive skills, including communication with patients with difficulties such as hearing loss, sight impairment, depression, speech problems, cognitive impairment, behavioral issues, or pain.
- Participate in the development of the service and organizational goals, aligning with the new model of care.
- Work effectively within the team, liaising with the multidisciplinary team (MDT) and other agencies through attendance at meetings and case reviews.
- Maintain accurate and timely records in accordance with Trust policy and HCPC guidance, reporting issues as needed.
- Provide specialist advice to team members, service users, and others regarding care plans, patient progress, and relevant matters.
- Support the planning and delivery of high-quality care on a day-to-day basis.
