Job summary

Mersey Care NHS Foundation Trust is seeking a Band 6 Physiotherapist to join the ICRAS North Liverpool & South Sefton Therapy team. The role supports adults 18+ with multiple co-morbidities, contributing to Urgent Community Response, intermediate care and hospital discharge pathways.

Responsibilities

• Undertake holistic assessments and devise individualized treatment plans through discharge.
• Obtain informed consent and engage patients in their therapy journey.
• Prioritise physiotherapy referrals by need, risk and capacity.
• Plan and deliver patient-centred individual or group interventions with graded activity.
• Monitor, evaluate and adjust treatment to measure progress.
• Provide clinical leadership and expertise within the team.
• Liaise with families, carers, GPs and other professionals.
• Develop risk management plans for service users.
• Monitor standards in planning and delivering physiotherapy activities.
• Lead or participate in service user reviews.
• Develop care plans to enable optimal health and independence.
• Demonstrate understanding of capacity and the Mental Capacity Act.
