An exciting opportunity has arisen to work in EPUT's Stroke Early Supported Discharge Team within South East Essex.

If you are interested in Stroke and want to support stroke survivors in reaching their full potential at home and in community settings, this job is for you. We are seeking a dynamic, keen, and resourceful Physiotherapist to join our well-established Early Supported Discharge Team.

This role will broaden your knowledge as we use an interdisciplinary approach, working closely with Speech and Language Therapists, Occupational Therapists, Clinical Psychologists, Nurses, and rehabilitation assistants to deliver high-quality stroke rehabilitation in patients' homes. There may also be opportunities to work across the community stroke pathway, including providing in-reach therapy into the Stroke rehabilitation unit (CICC).

1. Planning and delivering high-quality stroke-specific physiotherapy within the ESD team and other community stroke services.
2. Managing a caseload of clients discharged early from hospital, coordinating discharge, and implementing assessment and treatment plans.
3. Working within an interdisciplinary framework to provide integrated stroke rehabilitation care programs for patients on the community stroke pathway, supporting rehabilitation assistants.
4. Contributing to planning, development, and evaluation of clinical services within the ESD team and community stroke services, including managing defined projects.
