Job summary

Sanctuary Personnel are helping to recruit a Band 7 Pharmacist for Specialist Older Persons Medicine in Portsmouth. The role at Queen Alexandra Hospital involves leading the Older Persons Medicine pharmacy service and providing expert clinical support for frailty and elderly care.

Qualifications

• Master’s or Bachelor’s degree in Pharmacy with GPhC registration.
• Certificate in Clinical Pharmacy; Diploma in Clinical Pharmacy completed or in progress.
• Non-Medical Prescribing qualification or willingness to work towards it.

Responsibilities

• Lead the Older Persons Medicine pharmacy service at Queen Alexandra Hospital.
• Provide specialist clinical pharmacy support to frailty and elderly care patients.
• Support development and delivery of pharmacy services within frailty pathways.
• Collaborate with multidisciplinary teams to enhance patient care pathways.
• Develop integrated pharmacy services bridging primary and secondary care.
• Offer expert advice on frailty medicine and polypharmacy.
• Lead deprescribing initiatives on frailty and elderly wards.
• Deliver expert clinical pharmacy services to various wards.
• Participate in dispensing and carry out pharmaceutical duties.
• Support development of integrated neighbourhood teams and primary care links.
• Tutor, mentor, and develop junior pharmacists.
• Engage in service development and pathway redesign projects.
• Contribute to education and training programmes within the department.
