A regional healthcare provider is looking for a Senior Pharmacist for their Hospital at Home service in Northallerton. This role involves leading clinical pharmacy services and conducting patient visits to ensure optimal medication management. Candidates must be qualified pharmacists with experience in clinical settings and should be able to work independently and in teams. The position offers competitive pay ranging from £55,690 to £62,682 per annum, pro rata, along with leadership training opportunities.

Would you like to play a part in the Hospital at home service in the beautiful location of Hambleton and Richmondshire.
We have a rare opportunity for a 9-month maternity leave cover to lead and oversee clinical pharmacy input to the Urgent Community Response (UCR) and virtual ward team to deliver care to the virtual ward, community beds and Urgent Care Response teams.
You will contribute to patient care both within the unit base setting, and via patient home visits where appropriate.
Lead on medication provision and delivery across organisational boundaries.
Complete Medication reviews, medicines reconciliation and pharmaceutical care plans for patients admitted to the virtual ward.
Successful candidate must have access to a vehicle for work purposes to meet the requirements of the role.
You will work as an independent prescriber, optimising medicines and promoting appropriate, evidence-based, and cost-effective prescribing and deprescribing as part of the wider hospital at home team.
You will be using your clinical skills and knowledge in investigations, analysing, and implementing actions for medication related incidents within the virtual ward.
The provision of specialist clinical pharmacy knowledge to a range of health and social care professionals on the safe prescribing, handling, and administration of medicines.
