Job overview

King’s College Hospitals NHS Foundation Trust, along with The Hurley Group, has developed an integrated approach to medicines optimisation and improved community management of long-term conditions.

This innovative role within the Hurley Group involves working with eConsult to process patient queries and supporting medicines management through collaboration with general practice, under the guidance of a Lead Clinical Pharmacist. The role includes providing specialist advice, conducting full patient-centered medication reviews, especially for elderly patients with complex polypharmacy.

Main duties of the job

  • Participate in the multidisciplinary team to deliver high-quality, cost-effective, patient-focused pharmaceutical care.
  • Provide interface and discharge liaison support within the Care Group.
  • Lead clinical pharmacy services across various sub-specialties, ensuring high standards of input.
  • Act as a clinical role model and supervise rotational staff at ward level.
  • Audit medicine usage to promote cost-effective prescribing.
  • Collaborate with general practice staff to promote the role of GP practice pharmacists and raise awareness of available services.
  • Work within the eHub to process medication eConsults, reconcile medications, and manage medication workflows virtually across primary care sites.
  • Support medicines optimisation and prescribing initiatives aligned with organizational objectives.
  • Provide training to practice staff on medication issues, policies, and guidelines to promote safe and effective prescribing.
  • Contribute to improving electronic discharge information and communication processes.
