Job Overview

King’s College Hospitals NHS Foundation Trust, in collaboration with The Hurley Group, has developed an integrated approach to medicines optimisation and community management of long-term conditions.

This innovative role within the Hurley Group involves working with eConsult to process patient queries and supporting medicines management within general practice, under the guidance of a Lead Clinical Pharmacist. The role includes providing specialist advice, conducting comprehensive patient-centered medication reviews, especially for elderly patients with complex polypharmacy.

Main Duties
  1. Participate in a multidisciplinary team delivering high-quality, cost-effective, patient-focused pharmaceutical care.
  2. Support interface and discharge liaison within the Care Group’s wards and units.
  3. Lead clinical pharmacy services across various sub-specialties, ensuring high standards of input.
  4. Act as a clinical role model and supervise rotational staff at ward level.
  5. Audit medicine usage to promote cost-effective prescribing.
  6. Collaborate with general practice staff to promote and raise awareness of the GP practice pharmacist role and available services.
  7. Process medication eConsults, reconcile medications, and manage workflow across primary care sites via the eHub in a virtual setting.
  8. Support medicines optimisation initiatives and prescribing policies aligned with service objectives.
  9. Provide training to general practice staff on medication-related policies to promote safe and effective prescribing.
  10. Contribute to improving electronic discharge processes and communication.
