Pilgrims Hospices in East Kent is seeking an experienced Palliative Specialist Nurse to join our Thanet Hospice team. You will provide expert palliative and end-of-life care for patients with life-limiting illness, working autonomously within a multidisciplinary team.

You will lead complex assessments, advise on symptom management, plan care with patients and families, and contribute to service development, governance and education, including audits and quality improvement projects.

Formación

• Significant experience as a registered nurse.
• First degree in nursing/palliative care or associated clinical field or equivalent.
• Formal mentorship qualification including as a practice supervisor or assessor.
• Must hold valid, unrestricted right to work in the UK without employer sponsorship.

Responsabilidades

• Provide specialist palliative care assessment and support to patients with complex needs.
• Undertake holistic assessments considering physical, psychological, emotional, social, cultural and spiritual needs.
• Develop, implement and evaluate individualised care plans in partnership with patients, families and professionals.
• Prescribe medicines safely where qualified and authorised to do so, in line with local policies.
• Lead and participate in multidisciplinary team meetings and share specialist nursing knowledge.
