The role involves working within the acute therapy delivery team to support patients' recovery at home, ensuring continuity of care from hospital to community. An exciting opportunity has arisen to join Lancashire South Cumbria Foundation Trust’s (LSCFT) Central + West Network as a Senior Therapist.
The role will be within the acute therapy delivery team at Care Connexions, part of the Urgent Community Response, Virtual Ward, and Home First teams. These teams comprise physiotherapists, occupational therapists, and technical instructors, focusing on admission avoidance and early supported discharge from hospital by assessing patients' functional and care needs in their home environment, following the discharge to assess model.
The teams collaborate with social care providers to deliver integrated health and social care, ensuring appropriate intermediate care services are commissioned based on patient needs.
You will enhance patient and family experience during discharge, promote continuity of care from hospital to community, and support recovery at home after needs assessment.
Occasionally, you may be asked to cover broader community network roles, including triage work within Care Connexions' single point of access.
Part of this role involves clinical triage to support timely discharges and appropriate service commissioning, as well as admission avoidance triage.
We seek a dynamic, enthusiastic, and forward-thinking therapist with excellent communication skills, sound assessment abilities, and the capacity to work autonomously and within a team.
Hours may vary over 7 days between 08:00 and 20:00, depending on service needs. Part-time hours will also be considered.
This position is subject to an enhanced DBS disclosure.
