We are currently looking for aBand 6 Occupational Therapist, to join our inpatient therapy team on Longleat ward at Warminster community Hospital in Wiltshire. This role offers an excellent opportunity to work as part of a multi-disciplinary team delivering holistic, patient centred and evidence-based care.
HCRG Care Groupis a forward thinking, innovative service provider delivering treatment and care for a wide-ranging population across a mixture of rural and suburban settings. The service provides support to three acute trusts and is constantly developing new services to provide the best level of care to the people of Wiltshire.
Longleat wardis a 25 bed IP ward with a multi-disciplinary approach to the patient pathway. Patients are admitted to our ward from our 3 acutes for ongoing care. Conditions can be varied and complex and include orthopaedics, neurological, musculoskeletal, frailty, dementia and end of life.
To be successful in the post you must demonstrate a keen interest in care of the older person and be able to deliver high quality patient rehabilitation and discharge planning of complex patients within a community hospital ward environment using evidence-based practice. You must be able to prioritise and organise your caseload, support in the development of B5 OT staff, students and be experienced in supporting and delegating to therapy assistants.
You will be an excellent communicator, enthusiastic, flexible, work autonomously using strong clinical assessment and reasoning skills as well as supporting and leading on complex discharges.
