Qualifications

  • Senior band 6 Occupational Therapist role in Housing team.
  • Strong clinical skills with experience in community pathways advantageous.

Responsibilities

  • Undertake comprehensive assessments and manage clinical risks.
  • Represent patients at MDT meetings for coordinated service delivery.
  • Maintain accurate patient treatment records in line with standards.

An exciting opportunity has arisen for a senior band 6 Occupational therapist to join an established and expanding team working in Housing team in North East Lincolnshire, assessing individuals for major adaptations via disabled facilities grant process or for rehousing support.

Description:

  • Undertake comprehensive assessment of patients including those with diverse or complex presentations or multiple pathologies; use advanced clinical reasoning skills, including the assessment of cognitive and perceptual impairment, interpreting these assessments and providing appropriate recommendations, considering appropriateness of a DFG recommendation or rehousing or equipment or additional rehabilitation where appropriate.
  • Assess and manage clinical risk within own and delegated patients
  • Assess patient understanding of proposals; gain valid informed consent and work within a legal framework with patients who lack capacity to consent
  • Represent the Therapy service and/or individual patients at MDT meetings to ensure the delivery of a co-ordinated multidisciplinary service.
  • Work as a lone practitioner with telephone support from a senior Occupational Therapist if required
  • Be responsible for maintaining accurate, comprehensive patient treatment records in line with COT Standards of Practice and. To comply with COT Code of Ethics and Professional Conduct and national and local policies and procedures
  • To be aware of Trust policies and statutory requirements such as Health and Safety, Equal Opportunities, COSHH, Fire Safety, Complaints and Incident Reporting

Skills:

  • Strong clinical skills in assessing and treating a range of conditions.
  • Ability to develop and implement effective rehabilitation and discharge plans.
  • Excellent communication and teamwork abilities.
  • Demonstrates evidence of recent CPD and an ability to reflect on experiences
  • Experience with community-based pathways is advantageous.
