This exciting opportunity presents a chance to work within the Central Crisis Response Team and work closely alongside the newly established Frailty Hospital at Home team, focusing on hospital avoidance and caring for patients within their own home environment.
The post holder will be responsible for providing a rapid assessment of patients who have urgent care needs in the community. The team has an experienced multidisciplinary group, including Advanced Clinical Practitioners. You will be expected to support members of the team, act as an autonomous practitioner, and champion the role of Occupational Therapy within urgent care. The ability to problem solve and think on your feet is integral to the day-to-day running of a Crisis Team.
The purpose of the team is to avoid unnecessary hospital admissions. As a team, we initiate, assess, and treat patients with complex, acute, and specialist needs to enable them to remain safe and stable within their own homes. Referrals are received from NWAS, GPs, A&E, and community services. Together, we work with primary care, social, and voluntary sectors to collaborate on services, support, and care for the patient.
