Job summary

Royal Surrey NHS Foundation Trust in Guildford is seeking an experienced Senior Nurse to join the Urgent Out of Hospital Care team. You will deliver urgent and planned care within the community, with autonomy and support from a multidisciplinary team, including two-hour response visits, assessments and care planning.

You will lead in UCR and Hospital at Home services, ensure safe staffing and contribute to service development while maintaining patient-centred care outside hospital settings.

Qualifications

• NMC registration is required.
• Practice supervisor/Practice Assessor qualification is essential.
• Evidence of a range of clinical competencies appropriate for the role.
• Degree level qualification in health-related subject or equivalent experience.

Responsibilities

• Deliver urgent two-hour response visits, comprehensive assessments, clinical reviews and care planning.
• Provide clinical leadership within the Urgent Community Response and Hospital at Home teams, ensuring high-quality, patient-centred care.
• Manage day-to-day service delivery, supervise staff, ensure safe staffing and contribute to quality improvement and governance.

Degree level qualification in relevant health training/experience
