An exciting opportunity has arisen to recruit a Band 6 Senior Nurse Practitioner to join an established nursing team within the Greater Manchester Specialist Support Team (GMSST), which provides a service across Greater Manchester to service users with a learning disability and/or autism who present with risk of hospital admission, placement breakdown or forensic risk.

The post will be as part of the Greater Manchester Specialist Support Team (SST) based in Eccles. The GM SST is a multidisciplinary team consisting of Nursing, Occupational Therapy, Social Work, Psychology, Speech and Language Therapy, and Support Workers. You will be expected to work in all areas of Greater Manchester and liaise with the local Learning Disability, Mental Health and Autism services to develop proactive and preventative risk mitigation support, as well as discharge support for current inpatients.

We deliver a variety of nursing assessments and interventions to enable a shared understanding of an individual’s needs, combining and understanding of physical health, mental health and risk. We aim to reduce risk of unnecessary hospital admission and facilitate discharge (from acute and secure settings) at the earliest opportunity and reduce restrictive practice where possible. We also offer advice and consultation and training to external agencies.

To support the delivery of high quality specialist assessment and intervention in the development of effective clinical services to meet the complex needs of people with behaviour that challenges particularly those whose behaviour presents serious risks to the community.

Working as part of a multi-disciplinary team, the senior nurse practitioner will be responsible for managing their own caseload of individuals at risk of hospital admission, placement breakdown or criminal justice contact.

1. To carry a clinical caseload for the SST with individuals who present behaviour that challenges or where there is a risk of offending.
2. Under direction undertake a thorough assessment of new referrals using a range of tools.
3. Under direction provide comprehensive risk assessment of offending and/or other complex behaviours.
4. To carry out initial assessments with service users who are referred to the SST.
5. To develop Risk Management Plans including strategies and recommendations.
6. Contribute to the development of a formulation of offending behaviour and/or risk.
7. To support the implementation of specialist therapeutic
interventions.
8. To evaluate the outcome of any interventions.
9. Develop detailed service specifications when planning discharges from hospital settings or when seeking alternative placements in community settings.
10. To disseminate the Positive Behavioural Support model in delivery of care.
11. To provide advice and support to staff teams to maintain consistency in the overall management and response to risky or challenging behaviour.
12. To undertake on occasions urgent or emergency assessments, including risk assessments.
13. To provide recommendations for a gate keeping assessment where appropriate.
14. To support and lead on the development of contingency plans for crisis and relapses.
15. To organise own workload and work schedule.
16. To provide clinical and managerial supervision to junior members of staff.
17. To act as supervisor, assessor and mentor for students, nurses and junior staff.
18. To facilitate the education of service users, relatives and other agencies concerned with provision of learning disability services in the community.
19. To be involved in the development of training / information resources for partner agencies in the area of Learning Disabilities and Mental Health difficulties.
20. To ensure Safeguarding requirements are upheld.
21. To organise and chair regular staff meetings and other forums as appropriate.
22. To ensure effective communication between Health Services, Criminal Justice Services, Social Services and the Third Sector, and the co-ordination of other services as appropriate.
23. To undertake duties as may be delegated by the Team Manager.
24. To utilise theory, evidenced based literature and research to support practice.
25. To contribute to the evidence base by leading and participating in research and audits whenever possible.
26. To work across a variety of services and contexts, including prisons, secure services and psychiatric services as well as service users own homes in the community.
