Job summary

Mersey Care NHS Foundation Trust invites applications for a community mental health nursing role at South Sefton Neighbourhood Centre. You will provide high quality, evidence-based care within inpatient and community services, guided by the recovery model and a commitment to social inclusion.

You will act as a named nurse and mentor to students, lead within a multidisciplinary team, and collaborate with primary and secondary care services to support dual diagnosis and complex needs.

Qualifications

• Provide high quality, evidence-based care within Division for service users and their carers within inpatient and community services.
• Work in a socially inclusive way and in line with the recovery model.
• Act as named nurse/keyworker to service users undertaking assessment, planning and evaluation and review of care, within a recovery focused model.
• Participate as an active member of the multi-disciplinary team.
• Provide advice, expertise and leadership within the team.
• Supervise care for service users presenting with complex and high-risk needs.
• Work autonomously, reaching clinically appropriate decisions based on information received and using the knowledge and experience of the multi-disciplinary team.
• Be qualified to act as a mentor for students.
• Support and improve the physical health outcomes of service users within the Community Mental Health Team through assessment, monitoring, health promotion and liaison with primary and secondary care services.
• Contribute to the assessment and management of service users with co-existing Mental health and substance misuse needs (dual diagnosis), working Collaboratively with partner agencies.
