Overview

The Community Enhanced Rehabilitation Team are looking to recruit a Band 6 Senior Nurse Practitioner full time on a permanent contract. We are a supportive, well-resourced and dedicated team and there will be opportunities for professional development within the role. The successful candidate will be a Registered Nurse who will work alongside a large multidisciplinary team. The role is varied and may include covering duty, acting as a keyworker to coordinate care and undertaking routine assessments. Full induction, training and shadowing opportunities will be provided to the right candidate. This is an exciting opportunity to be part of a newly forming team delivering high quality specialist care. Interview week commencing 13 October 2025.

Job responsibilities

CLINICAL

  • Assist in the development and implementation of integrated care pathways, including appropriate service user referrals and models of care.
  • Participate in case supervision to monitor named primary nurses standards of service user care and documentation.
  • Provide/receive regular clinical supervision as per Trust policy.
  • Ensure the clinical environment promotes service users health, safety and well-being, preserving personal dignity, privacy, beliefs.
  • Ensure record keeping complies with Professional and Trust standards.
  • Ensure safe management of medicines and adhere to safe practice and principles of administration per NMC code and Trust Medicines Code.
  • Work collaboratively with the Multi-disciplinary team and promote positive intra-agency working.
  • Participate in the referral process and assess individuals in line with the agreed care pathway and complete quality reports where appropriate.
  • Assess care needs and develop, implement and evaluate programmes of care without supervision; act as Care Coordinator as required.
  • Participate in setting standards of service user care based on research findings.

MANAGERIAL

  • Undertake delegated responsibilities as agreed with Clinical Team Manager.
  • Support clinical and operational practices and procedures as appropriate.
  • Participate in the Duty Rota according to locality structures.
  • Assist in management and coordination of all services delivered to service users within the clinical area.
  • Support the implementation, monitoring and updating of an operational policy for the clinical area.
  • Ensure Clinical Governance principles are incorporated in service delivery and maintain a high quality service based on clinical standards.
  • Ensure service users are cared for within Mental Health Act status and adhere to the MHA 1983 and Code of Practice (liaison with Home Office for restricted service users if required).
  • Ensure adherence to Effective Care Coordination; adhere to Trust policies and procedures.
  • Assist in developing local protocols to maintain high standards of service delivery.
  • Support staff participation in developing new ideas to improve care and service delivery.
  • Provide effective leadership to the designated team and supply required statistical information.
  • Maintain communication processes within the team; assist in investigating complaints and incidents as per Trust policy; participate in Serious Untoward Incident investigations as required.
  • Coordinate utilities, equipment and services and report issues to managers; provide management supervision for delegation of duties to ensure consistent care.
  • Ensure nursing practice adheres to Trust policies; participate in managerial supervision and in managing staff performance and attendance.

EDUCATION AND DEVELOPMENT

  • Ensure all relevant staff have up-to-date Personal Appraisal and Personal Development Plans.
  • Coordinate staff for appropriate courses including mandatory training according to service need and development plans.
  • Organise and undertake teaching where appropriate; promote evidence-based practice.
  • Coordinate preceptorship and mentorship; participate in regular supervision and reflective practice; commit to continued development.
  • Develop skills and contribute to the development of others.

RESEARCH AND AUDIT

  • Ensure staff awareness of Clinical and Corporate Governance and the need for research-based practice.
  • Undertake and participate in audit, quality assurance programmes and research as directed.
Qualifications and experience

Qualifications

  • Essential: Registered on the appropriate part of the NMC register and hold a current NMC registration or a relevant professional qualification (to be maintained).
  • Desirable: Post-registration training in relevant field; Leadership/Management training.

Experience/Knowledge

  • Essential: Recovery Model; Mental Health Act (1983); Effective Care Coordination; Relevant national policies and guidance; Evidence-based and reflective practice; Mental Capacity Act; Vulnerable Adults Procedure and Child Protection; Safety, Privacy and Dignity Guidelines; Experience of mentorship, supervision and coordination of staff; Experience of Care Planning; Working in a multiagency framework; Change management experience; Problem-solving approaches.
  • Desirable: Knowledge of Trust objectives; Experience as Nurse in Charge/Shift Coordinator; Positive team leadership.
