Berkshire Healthcare NHS Foundation Trust is seeking a Band 6 Senior Mental Health Practitioner to join the Slough Community Mental Health Service at New Horizons. You will work autonomously with adults with complex mental health needs, delivering assessments, care planning, and evidence-based interventions within a supportive multidisciplinary team.

Qualifikationen

• Hold and manage a defined caseload for adults with serious and enduring mental health needs.
• Complete comprehensive mental health assessments, formulations, care and crisis plans.
• Coordinate evidence-based, recovery-focused interventions in various settings.
• Collaborate with multidisciplinary teams and partner agencies to develop coordinated health and social care packages.
• Undertake Care Act 2014 and safeguarding responsibilities as needed.
• Provide clinical leadership and supervision to junior staff and students.
