Senior Mental Health Practitioner - Band 6:

Our Crisis Resolution Home Treatment Team is part of the Crisis Care Centre, at Harplands Hospital, Stoke-on-Trent. This has been designed to the highest level of specification. This modern, purpose-designed unit will be the central point for all those in need of a Crisis Assessment/Intervention, within the teams of the Crisis Care Centre.

Main duties of the job

The post holder is responsible for the delivery of care and for the supervision of others who are carrying out all aspects of the caring process. The post-holder will assist in setting and maintaining standards of care, participate in effective management of the team and unit, and will also maintain effective integration with the multi-disciplinary team, facilitating a collaborative approach to patient-focused care.

Job responsibilities

  • Act in accordance with the Nursing & Midwifery Council (NMC) code of professional Conduct or relevant professional body.
  • Perform all role aspects in line with Trust policies and national guidelines.
  • Participate in assessing care needs and developing, implementing, and evaluating individual care plans.
  • Provide specialist advice and act as a role model to staff.
  • With the Team Manager, set, maintain, and evaluate standards of care 24/7.
  • Liaise with multidisciplinary team and external agencies for patient care.
  • Ensure care activities are research-based and current with field developments.
  • Contribute to innovative practices and departmental audits.

Person Specification

Essential Qualifications:

  • A registered mental health practitioner with a degree in nursing, social work, occupational therapy, or equivalent experience.

Experience:

  • Post-qualifying experience in the core profession.

Additional Requirements
