Senior Mental Health Practitioner

We have a fantastic and rare opportunity for a practitioner to join our friendly, dynamic, and busy multi-disciplinary team. We provide comprehensive assessment and intensive home treatment which involves high quality and recovery focused care to adults experiencing a mental health crisis. We offer an alternative to hospital admission and support with early discharge and therefore work closely with our inpatient and community team colleagues to ensure an effective transition of care across the pathway. You can expect to work for a robust, well-established team with strong values in a positive working environment. We are heavily community based and cover the beautiful New Forest National Park and surrounding areas. You will have the chance to develop your leadership skills, access exciting training opportunities and enhance your knowledge of psychological interventions. As a team we prioritise staff wellbeing and progression and offer regular supervisions, reflective practice and in house education sessions.

We offer a 24/7 service, 365 days a year however we are looking for a motivated and highly skilled practitioner to work between 8am and 10pm.The role involves a mixture of being office based, remote working and working out in the community. We are happy to discuss flexible working.

Main duties of the job

To contribute to the provision of intensive home based treatment, as an alternative to hospital admission, providing a 24 hour service across the acute care pathway; To work proactively in accordance with the principles of social inclusion, promoting independence and recovery.

To undertake ongoing assessment, implementation and evaluation of care needs of a changing caseload of service users within their home environment.

To work closely with the inpatient units ensuring the effective transition of care across the acute pathway.

Ensuring the regular attendance at ward rounds, professionals meetings to identify service users who may benefit from the Hospital at Home Service.

Liaise closely with Community Treatment Teams ensuring the safe discharge of service users following a period of treatment with the Hospital at Home Service.

To work as a lead practitioner in managing the shift and a flexible, team based caseload

To mentor, supervise and teach qualified and unqualified staff including students.

To lead the care for a specified group of service users.

To undertake supervision and appraisal for a defined number of junior staff.

To work with other members of the multi-agency team to plan, monitor and review the effectiveness of the risk management plan.

To ensure effective communication with clients, relatives and carers, visitors, staff and accurately record all contacts with service users in case notes in line with the Trust's record keeping policy.
