Overview

Senior Mental Health Practitioner to join the Primary Care team at HMP Wakefield, a category A prison in West Yorkshire.

Hours: 7 day service. Monday-Friday 08:00-16:00 or 08:00-18:00, Saturday-Sunday 08:30-16:30. 18:00 finish. Weekend duties and bank holidays are shared across the team. This is a full-time role with an annual salary up to £47,466 per annum FTE, depending on experience.

Main duties / Responsibilities
  • Responsible for assessment and ongoing management of patients’ mental health needs
  • Work collaboratively with the multi-disciplinary team as an autonomous practitioner managing a caseload of patients, assessing their physical, social and psychological needs to develop a holistic health care plan
  • Contribute to the nursing strategy and vision to provide outstanding evidence-based care, seeking and acting on patient feedback to personalise care
  • Deliver non-judgemental care, build therapeutic relationships and maintain professional boundaries
  • Participate in assessment, planning, implementation and evaluation of patient care, following policies and safeguarding procedures

Qualifications / Experience
  • Occupational Therapist or Social Worker - HCPC / NMC Registered
  • Experience of working within mental health settings and understanding this patient group
  • Evidence of Continued Professional Development
  • Experience of working collaboratively with other agencies and stakeholders in a multidisciplinary environment

Experience in a hospital, community, or prison environment is desirable but not essential.
