Main Duties
  • Undertaking assessments of new clients and providing care-planned interventions to your caseload.
  • Supporting the team as it engages with four primary care networks at different stages of implementing Community Mental Health Framework developments, enhancements to provision for clients in residential care, and services for those living with frailty in their own homes.
  • Contributing to staff career development and wellbeing within the team and Devon Partnership Trust.
  • Based at Bideford Hospital with weekly MDT meetings; additional desk space at Barnstaple Health and Wellbeing clinic. Clients are seen in their homes, clinic settings, and via remote technology.
