Job summary

Norfolk and Suffolk NHS Foundation Trust is seeking a Senior Mental Health Practitioners (Nursing/OT/SW) for a year-long secondment within the Adult Community Mental Health Team based in Bury St Edmunds. The post involves managing a diverse caseload, delivering clinical interventions, and collaborating with a multidisciplinary group to plan holistic care.

Qualifications

• Registered Mental Health or Learning Disability Nurse, Occupational Therapist OR Social Worker with the relevant professional registration.
• Experience in the delivery of clinical interventions.
• Ability to use clinical assessment tools and outcome measures effectively and collaboratively.

Responsibilities

• Manage a complex caseload under the Care Programme Approach.
• Undertake initial urgent/routine assessments for the service.
• Attend MDT meetings with Consultants, Nurse Practitioners and Allied Health Professionals.
• Attend outpatient clinic appointments and conduct home visits in the community.
• Engage in holistic goal setting and treatment planning with service users and carers.
• Provide supervision and mentorship to junior staff and contribute to service development.

Main duties of the job

Working in a busy and dynamic Adult Community Mental Health Team means every day will be different. You will be involved in:

• Managing a complex caseload for those under The Care Programme Approach
• Undertaking initial urgent/routine assessments on behalf of the service
• Attending multi-disciplinary team meetings with our Consultant Psychiatrists, Nurse Consultants, Clinical Team Managers, Senior Practitioners, Assistant Practitioners and Allied Health Professionals including Consultant Clinical Psychologists, Clinical Associate Psychologists and Assistant Psychologists
• Attending outpatient clinic appointments
• Undertaking face to face visits in the community setting including home visits
• Working with service users and their families/carers to care plan holistic goal setting and engage in treatment planning and working in collaboration with internal/external stakeholders
• Medication administration (if qualified to do so) with opportunities to enrol in local university training programmes in medication management
• We also works with needs types and have many evidence based work books to enhance the Service user experience.
• Involved in the clinical leadership decisions as part of the senior leadership team to make sure that KPIs are met and team goals are being worked towards
• Supervision and mentorship of junior staff members
