Job summary

NSFT is seeking a skilled mental health practitioner to join the Memory Assessment Team for Care Homes (MAT-CH). You will assess older adults in care homes, contribute to diagnostic formulation, and support care home staff with person-centred care and non-pharmacological management of dementia.

The role requires driving and travel across Norfolk and Waveney, with opportunities for hybrid working and ongoing professional development. Registration in a relevant profession is essential.

Qualifications

• Registered Mental Health or Learning Disability Nurse, Occupational Therapist or Social Worker with the relevant registration
• Practice Assessor and Practice Supervisor training (NMC registrants) or Practice Educator (AHP & Social Worker Registrants)
• Ongoing professional development working towards degree level (if professional registration is following a diploma level course)

Responsibilities

• Undertake comprehensive mental health and cognitive assessments for care home residents with suspected dementia.
• Provide specialist advice and support to social care staff, families and carers.
• Medication review and management; monitor treatment effectiveness and tolerability.
• Collaborate with the multidisciplinary team to promote outcomes and high-quality dementia care.
• Engage in clinical audit, reflective supervision and continuing professional development.
