Longfield Hospice is seeking an experienced Registered Nurse to lead our Hospice at Home team in delivering compassionate end-of-life care in patients' homes across Gloucestershire. You will provide expert clinical leadership, oversee referrals, coordinate care with a multidisciplinary team, and support staff, patients, and families with assessment and guidance.

In this role you will help prevent unnecessary hospital admissions, contribute to service development and governance, and supervise HCA

Responsabilidades

• Lead and support Hospice at Home team to deliver high-quality care in patients' homes
• Manage referrals and coordinate care with multidisciplinary partners
• Provide expert clinical leadership, assessment, and guidance to staff, patients, and families
• Help prevent unnecessary hospital admissions, enabling patients to be cared for at home
• Support service development, governance, staff training, and clinical standards
• Line manage HCA and RN teams, providing supervision and performance reviews
