Job Description:

The role of Senior Hospice at Home Paramedic/Nurse is to provide high quality specialist palliative nursing care, skills, advice, and support to patients, carers, and those important to the patient. This role supports the delivery of planned and unplanned care across Rennie Grove Peace Hospice.

You will work across shifts from 7am to 9.30pm. Although based in the community, post holders may occasionally work in other Rennie Grove Peace Care settings to ensure adequate staffing and gain wider hospice care experience.

Key responsibilities include:

  • Participate in the development of the community Hospice at Home care model.
  • Work as part of a multidisciplinary team to provide high-standard, individualized care and support.
  • Manage daily patient allocations responsibly.
  • Ensure excellent standards of clinical practice and support.
  • Deliver specialist palliative nursing care for planned and unplanned cases, including multi-disciplinary approaches for higher acuity patients with virtual and in-person reviews at their homes.
  • Collaborate with the wider hospice team and system partners to support care delivery and help patients remain in their preferred place of care.
  • Enhance patient outcomes and experiences through physical and emotional support and care coordination.
  • Participate in audits and case reviews to improve the hospice at home service.
  • Act as a community ambassador for Rennie Grove Peace Hospice Care, promoting our values and the care we provide.
  • Support service delivery across the geographical area as required.
