Job summary

Stivic Care Services LTD in domiciliary care is seeking a dedicated Care Assessor to join our care team. You will assess the needs of service users, develop person-centred care plans, and ensure care delivery meets individual needs and regulatory standards.

You will work in service users' homes, collaborate with families and healthcare professionals, maintain accurate records, and participate in audits and rotating on-call duties to safeguard quality and dignity.

Qualifications

• Experience in care assessment or senior care within domiciliary services.
• NVQ/QCF Level 3 in Health and Social Care or working towards it.
• Knowledge of CQC standards and Care Act principles.
• Full UK driving licence (essential); access to a vehicle (desirable).

Responsibilities

• Conduct initial and ongoing care assessments in clients' homes.
• Develop and review person-centred care plans.
• Carry out regular reviews to monitor care arrangements.
• Escalate safeguarding concerns and ensure regulatory compliance.
• Maintain accurate records on the care management system.
• Participate in rotated out-of-hours on-call duties.
