A leading health service provider in Bristol is seeking a motivated individual to join the Health Link Team. The role focuses on supporting vulnerable individuals facing multiple disadvantages, facilitating access to healthcare services, and working closely with partner agencies. Job summary: We are seeking a motivated individual who can demonstrate the skills required to develop trusting relationships and support individuals experiencing multiple disadvantages and facing significant systemic barriers to accessing and engaging with support and healthcare.
The successful candidates will be working within the Health Link Team alongside partner teams within the Homeless Health Service as well as a wide range of external agencies coordinating tailored support, identifying creative solutions, and empowering individuals to access and engage with health services more independently.
The Health Link team works with people who are homeless, at risk of repeat homelessness or who are vulnerably housed in order to improve their health, recovery and maximise engagement with healthcare and related services. The team specialises in working with people who experience chronic, complex health needs and multiple disadvantages, and who face systemic challenges which impact on engagement with health services. Health Link works with individuals who are frequent attenders of A&E services.
Second Step is a leading mental health provider in Bristol, North Somerset and B&NES. Our aim is to deliver Recovery and Wellbeing opportunities for adults with mental health and other support needs to achieve their hopes and ambitions.
Carry out role adhering to Second Steps policies, procedures, values, code of conduct and with a Recovery orientated approach, and in line with relevant and Homeless Health Service procedures Work to achieve agreed targets and performance indicators for the service. Provide assessments of the health related support needs of vulnerable individuals who are homeless or at risk of homelessness, working in tandem with other members of the Homeless Health Service team. Provide a signposting, advocacy and referral service to other relevant agencies, including follow up liaison to facilitate engagement and progress. Generate support plans and provide time-limited casework, focussing on the health related support needs of service users and ensuring a multi-agency approach to complex cases.
