Job overview

The Clinical Nurse Specialist (CNS) will demonstrate a high level of practice in both direct patient care and in the ability to plan and develop services to support cancer care. Through the key skills of leadership, innovation, and clinical expertise, the CNS will facilitate and deliver patient-centred care, which is efficient and effective.

The CNS is a core member within the multidisciplinary team (MDT). They will work collaboratively with clinicians and service managers to promote, develop, review, and redesign services that are responsive to patients' needs in line with national and local objectives. The CNS is an autonomous practitioner and has the freedom to act to influence patient care and service provision.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Main duties of the job
  • Demonstrate leadership through clinical expertise delivering high standards of personalised care by acting as a key worker, identifying needs through holistic needs assessment, and co-creating a care and support plan with people living with cancer.
  • Undertake holistic needs assessment, care planning, and evaluation from diagnosis, through treatment, and beyond, ensuring high levels of autonomy and advocacy to deliver ongoing appropriate care for a good quality of life.
  • Lead on the development, implementation, and evaluation of Nurse Led Clinics to support Holistic Needs assessment via telephone or face-to-face.
  • Establish working relationships and communication links between primary, secondary, tertiary, and independent care providers to improve patient care and provide specialist clinical expertise, advice, and support when required.
  • Ensure adherence to relevant clinical guidelines, policies, and procedures within their scope of practice and specialty.
  • Act as the patient advocate within the site-specific Multi-disciplinary Team (MDT). Request appropriate clinical investigations to support patient assessment as deemed appropriate by their line manager.
